As outlined in DHSC’s guidance for general practice in England on accessing DHSC centrally supplied flu vaccines, each GP practice is required to complete a return to declare:
- whether you sourced DHSC supplied flu vaccines
- the manufacturers you sourced DHSC supplied vaccines from
- how many vaccines you administered, redistributed and destroyed
This is a mandatory return and should be completed by a GP partner of the practice.
If you have not received a link by email asking you to complete your 2020/21 DHSC supplied flu declaration, you can complete your declaration online.
The deadline to complete the declaration is 11.59pm on Friday 30 April 2021.
