Raising dm+d issues

For initial and routine enquiries, email us at dmdenquiries@nhsbsa.nhs.uk, or use the specific email addresses listed under the Contact us section of the dm+d homepage.

You will need to submit a dm+d issue notification form if you:

  • have a complaint
  • are dissatisfied with the service
  • believe there is a major safety issue, risk or something unacceptable

This form must be emailed to dmdenquiries@nhsbsa.nhs.uk.
