From August 2022 (for July 2022 prescriptions), pharmacy contractors will no longer need to print and submit a paper copy of your completed FP34C declaration with your paper prescription bundle.
Instead, we will send you a paper Account Identifier Document, along with your red separators and pharmacy address labels each month.
You must continue to submit your FP34C declaration via the Manage Your Service portal (MYS) by the 5th day of the following month in which the supply was made. This change to submission requirements means you can dispatch your paper prescription bundle before submitting your declaration through MYS.
You should place the Account Identifier Document alongside your paper prescription bundle before sending it to us for payment. The Account Identifier Document will allow us to easily identify which pharmacy has submitted their prescription bundle, which will help to speed up prescription processing.
If you lose your Account Identifier Document you will be able to download and print another copy from MYS.
The July 2022 Drug Tariff will be updated to reflect this change.
