There are certain stages which Potential Suppliers intending to deliver the COVID-19 vaccination service from their Healthcare Premises must go through.
The COVID-19 vaccination programme Site Sign-up Process for Suppliers document (the 'Guidance') sets out the stages you must go through to join the COVID-19 vaccination programme.
This process is for new Potential Providers or for existing Providers whose current contracts expire in March 2025. If you are one of these Providers, you must follow this process and need to submit a Response Document.
Existing providers whose contracts extend to 31 March 2026 do not need to submit a new Response Document. Please read through the Guidance to ensure you only submit a Response Document if you are required to.

Submitting Response Documents
Response Documents can be submitted from the launch of the process to the Final Response Deadline.
Due to the periodic nature of the COVID-19 programme and the time needed to onboard new sites, there will be Interim Response Deadlines to allow enough time for sites to be onboarded ahead of a new COVID-19 vaccination Campaign (see the Key Dates in the Guidance).
If there is still a need for further provision beyond that capable of being provided by this Site Sign-up Process, a separate procurement process may follow for the selection of any further Designated Sites or activity required. Additional guidance would be issued on this if needed.
Response Document
This is to be completed by all Potential Suppliers who are proposing a Designated Site from their Healthcare Premises.
Only complete one Response Document submission for your Designated Site on the system.

Standard Selection Questionnaire (SQ)
Only completed by Potential Suppliers who are not on the pharmaceutical list, or do not hold (or will not hold by the Commencement Date):
- an NHS Standard Contract (not including the contract that this may lead to)
- a GMS, PMS or APMS contract

Submitting your Response Document
How to submit your Response Document will depend on the type of contractor or Potential Supplier you are.
In all cases once you have submitted the Response Document you will receive a confirmation email. Keep a copy of this for your records, it will also explain how to access a copy of your submission.
For Manage your service (MYS) users, the email will be sent from manage.your.service@notifications.service.gov.uk, for SnapSurvey users it will be sent from noreply@online1.snapsurveys.com.
Providers whose existing contracts expire on 31 March 2026 do not need to re-apply.
Any provider who wishes to participate must submit a Response Document before 11:59pm on the 19th November 2024.
Pharmacy Contractors with MYS accounts
Login to your MYS account, select 'COVID-19 vaccination programme: Site Sign-up Process for Suppliers', then complete a Response Document for your Designated Site.
If you have management level user access on MYS and wish to submit Response Documents on behalf of all your sites, submit a Response Document for each of your pharmacies registered under your account one at a time.
You can save your entry at any time, but make sure you submit your Response Documents in order to be considered as part of the Site Sign-up Process.
All other Potential Supplier types (such as PCN groupings, Trusts wishing to vaccinate the public, independent sector, private pharmacies)
Submit Response Documents using this SnapSurvey.
You can save your entry part way through which will result in an email being sent to you with a link to your partially completed Response Document.
Submitting more than 10 Response Documents
Where an organisation represents multiple Potential Suppliers they may wish to submit Response Documents for each Potential Supplier on their behalf (for example a pharmacy or nursing home chain).
You can request a “Response Document (more than 10 Designated Sites)” Excel spreadsheet by emailing mys@nhsbsa.nhs.uk. Let us know how many Healthcare Premises you intend to include in your spreadsheet, and what type of Supplier you are (such as community pharmacy or other provider type).
Make sure you complete it and return it to mys@nhsbsa.nhs.uk in line with the Key Dates in the Guidance.
